Certified Nursing Assistant (CNA)
Job Description
North Kern South Tulare Healthcare District is looking for a Certified Nurse Assistant (CNA) who will assist in providing exceptional patient care to the residents of our Delano District Skilled Nursing Facility.
Duties/ Responsibilities:
- Assume the administrative authority, responsibility, and accountability of performing the assigned duties of a Certified Nursing Assistant (CNA).
- Provide nursing care which assures the mobility status, emotional, social and medical/ therapeutic needs of the Resident are met/ maintained on an individual basis at a level consistent with each Resident’s maximum potential to attain, and as may be directed by the Nurse Manager, Charge Nurse, Director of Nursing Services, the Administrator and the attending physician for the Resident.
- Provide direct nursing care to the Residents including aid in areas of Activities of Daily Living, such as, but not limited to : bathing, grooming, toileting, dressing, ambulation/transfer assistance, eating, positioning in and out of bed and/ or wheelchairs/Geri-chairs, in accordance with current existing federal, state and local standards, as well as the facility established policies and procedures.
- Respond to and care for the immediate and urgent needs of any resident, whether assigned to you or not.
- Communicate a complete picture of the nursing care rendered by you in all the required documentation, as well as sharing specific concerns, ideas regarding changes in Resident condition to the Charge Nurse.
- Communicate with coworkers to foster an environment supportive of teamwork and congeniality.
- Observe Resident areas to; 1) communicate changes in Resident status and/or conditions, 2) analyze situations for safety implications, 3) effectively manage time and organize duties (in the best and most efficient way), 4) give feedback and communicate new ides to supervisors, etc. 5) encourage autonomy in all Residents, so they may participate in decision making, their care and activities, at a level consistent with each Resident’s maximum potential to attain.
- Show respect in all activities to Residents, co-workers, supervisors, and visitors to ensure dignity, integrity and consideration are an integral part of the workplace.
- Assure a safe and secure workplace by performing all nursing care in a safe manner to ensure a minimum of accidents or injuries to self, Residents, coworkers, supervisors and visitors.
- Observe infection control practices in all aspects of Resident care procedures and equipment handling, to ensure a safe and healthy environment for Residents, employees and visitors.
- Practice time-management skills by organizing all tasks and duties in the best and most efficient manner possible, while ensuring all assigned Residents receive compassionate, safe and thorough care in a timely fashion, and all other duties are completed with the same considerations.
- Develop and maintain a good working relationship with other department personnel within the facility, to assure the nursing care can be properly planned and maintained to meet the needs of the Residents.
- Read and follow each Resident’s Plan of Care to ensure all nursing efforts work to ensure the needs of the Resident are met/ maintained on an individual basis at a level consistent with that Resident’s maximum potential to attain, and the direct Resident nursing care is coordinated with all other Resident services to achieve that end.
- Participate in the Resident Care Plan meeting as a member of the ID team, giving input as to the social, physical, mental aspects of new residents, or long term residents in your charge. Assist to create a Plan of Care that accurately reflects the current functioning level for each Resident.
- Must attend 12 mandatory in-service education programs regularly to fulfill safety, regulatory and certification requirements, a minimum of 24 hours of training per year.
- Support the Residents need for active and meaningful activities by transporting those who cannot transport themselves to scheduled events.
- Complete all daily charting requirements to ensure all current care and functional status information is communicated. These requirements include, but in no way reflect the entire list: CNA Flow Sheets, Intake and Output, BM schedule, Turning schedule, meal percentages, etc.
- Perform “beginning of shift” round with the oncoming CNAs and NAs, to pass on all necessary care information, concerning Residents that may have occurred on your shift.
- Rounds are to be made and “report” given to ensure that the oncoming shift knows the status of all assigned Residents.
- At a minimum, care rounds are made on all assigned.
- Participate in regularly scheduled meetings with the Director of Nursing Services to give input as to supply levels, problems and/ or suggestions, new ideas on any aspect of nursing services.
- Organize Resident’s room, clothing and possessions daily to promote order, cleanliness, and ease of resident care handling.
- Dispense appropriately clean laundry to Residents’ room.
- Pack up (or unpack) Resident belongings in preparation for (or as a result of) Pest Control treatment program.
- Admit new Residents by assisting with their belongings and itemizing them on an “Inventory” form.
- Orient New Residents to the facility by familiarizing them with their rooms, bathrooms, call bells, station, dining room, activity room, etc.
- Prepare Residents for discharge, as directed, by inventorying and packing belongings, and verifying with laundry that all items have been returned. Ensure that Resident is groomed, appropriately dressed, and prepared for discharge.
- Perform postmortem care, as directed, to prepare Resident for pickup by mortuary or other agency.
- Complete an inventory of belongings on appropriate form and communicate any problems to charge nurse.
- Verify with laundry that all belongings have been returned.
- Other related duties and responsibilities that may become necessary to assure that the needs of the facility are met.
- Maintained health records and certificates up to date.
Required Skills/Abilities:
- Must possess, as a minimum of a high school diploma or GED (high school equivalent) is preferred.
- Must possess an active Nursing Assistant Certification from the State of California.
- Ability to exercise tact, initiative, and good judgment in dealing with people.
- Ability to accept/follow supervision and work independently as needed.
- Must be able to read, write and speak and understand the English language.
- Must have excellent work ethic, patience tact, and enthusiasm.
- Must be fully vaccinated against COVID-19, including having received the booster shot.
Education and Experience:
- Active Nursing Assistant Certification from the State of California
Physical Requirements:
- Prolonged periods of standing and moving may be required.
- Must be able to lift up to 25 pounds at times.
Salary:
Salary: $17.25-$18.45 per hour
