Enhanced Care Management (ECM) Coordinator

Job Description

The Enhanced Care Management (ECM) Coordinator at Gloria Nelson Center (GNC Family Clinic) is the key point of contact for members exhibiting complex social and emotional needs. The role involves collaborating with the multi-disciplinary team to provide extended healthcare solutions beyond inpatient care or traditional primary care. The coordinator ensures better medical management and assesses the effectiveness of interventions, working closely with members, providers, and community resources.

Job Duties:

  • Engage and support members in the ECM Program.
  • Monitor treatment adherence through Care Plans aligned with provider recommendations.
  • Conduct health promotion and self-management training.
  • Maintain regular communication with members, including home visits if necessary.
  • Build and nurture relationships with internal and external stakeholders.
  • Assist with discharge planning and transition of care.
  • Coordinate care with case management staff and community providers during emergencies.
  • Attend medical appointments to ensure continuity of care.
  • Document interactions and maintain records of referrals to various support services.
  • Conduct psychosocial assessments and develop treatment plans collaboratively.
  • Perform other ECM-related duties as assigned.
  • Ensure HIPAA compliance and treat member information confidentially.
  • Adhere to local, state, and federal regulations.
  • Contribute to quality assurance/improvement activities.
  • Utilize Electronic Health Record systems efficiently.

Qualifications, Education, and Experience:

  • Graduate of an accredited nursing program with a current California LCSW license, LVN license or Certified Medical Assistant certification from NCCT or AMT.
  • Minimum of two years of care coordination experience.
  • Bilingual in English/Spanish preferred.

Physical Requirements:

  • Prolonged periods of setting at a desk and working on a computer
  • Prolonged periods of stand and moving
  • Must be able to lift to 50 pounds and more.

Salary:

  • Salary: $25-$35

Work Schedule

  • Monday through Friday, 8a-5p with occasional Overtime
